I thought it was a PHP module (not standard, so it is not likely in most hosting packages), published in pdf format.
An easier way is to fill each box with some unique token ~ ~ fname, lname ~ ~ ~ ~ Social Security number, etc., and use file io ReplaceString basic functions. There must be secured only to open the file as binary, not ASCII.
